About this notice
Under the Health Insurance Portability and Accountability Act (HIPAA), Cardiology Associates of Central Brevard is required to maintain a written Notice of Privacy Practices. This notice describes:
- How we use and disclose your protected health information (PHI) for treatment, payment, and health-care operations.
- Other situations in which we may use or disclose your PHI — for example, when required by law.
- Your rights regarding your PHI, including the right to inspect, request copies, request amendments, request restrictions, and receive an accounting of disclosures.
- How to contact us or file a complaint if you believe your privacy rights have been violated.
How to get a copy
You can obtain our current Notice of Privacy Practices in any of these ways:
In our office
A printed copy is available at the front desk. New patients receive a copy as part of their intake paperwork on their first visit.
By phone
Call (321) 726-0007 during office hours and we’ll mail or email you a copy.
Your privacy rights
You have specific rights regarding your protected health information. These include, in summary:
- Inspect and copy your medical records and billing records.
- Request a correction to information you believe is incorrect or incomplete.
- Request a list of certain disclosures of your information.
- Request a restriction on certain uses or disclosures.
- Request confidential communications (for example, asking us to contact you only by phone, not email).
- File a complaint if you believe your privacy rights have been violated. You may complain to us directly, or to the U.S. Department of Health & Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
